COVID-19 - **CRITICAL CDC ORDER UPDATE & UPDATED FORM**
Below is the most recent intake form for your evictions. In response to Supreme Court Administrative Order 2021-47 we are now required to make certain disclosures with respect to rental assistance agreements that may or may not be subject to the case. Please make sure you read this form carefully and answer all questions that are being asked in the form. If you entered into an assistance agreement with any agency for the receipt of rental assistance funds you will need to review that agreement carefully to ensure that you are compliant with the agreement. Some assistance agreements require judgments be vacated, eviction actions stayed, or evictions delays for a prescribed period of time. Each agreement is different.
We recommend you review them carefully and you send them to us for our review if you have any questions or concerns.
