COVID-19 - **CRITICAL CDC ORDER UPDATE & REVISED-UPDATED FORM**
Dear Clients,
Below is the new intake form that we have been using since April 1, 2021. Please make sure that your teams have a copy of this form and that it is filled out and submitted with each of your evictions. This form seeks information that we are now required to plead in our filings for evictions. This form is intended to ensure that all complaints are accepted by the court and not dismissed on a technicality. We understand that this presents another change to the process and appreciate your assistance as we continue to navigate the ever changing legal landscape with evictions in Arizona
